Botulinum toxin (Botox) has been used successfully in medicine for over 30 years, not only for aesthetic purposes, and is a drug with a very high level of therapeutic safety. Botox is used for muscular tension, eyelid spasms, the treatment of teeth grinding (bruxism) or the therapy of migraine. The principle is based on the targeted, dose-dependent relaxation of the underlying muscular overactivity, which largely causes the disorders and complaints described.

In aesthetic medicine, the relaxing effect on the muscles is also exploited, with the active ingredient injected in very small and highly diluted amounts. The face and neck region account for the largest share, with the therapeutic approaches focusing in particular on wrinkle prevention and wrinkle reduction. Botox is also used very successfully for excessive sweating in the armpits or to even out asymmetries.

Treatment concept

My goal is for you to be lastingly satisfied with your result while preserving your natural look. That is precisely why the first consultation and the analysis of your face and the region to be treated are of particular importance. Drawing up a treatment concept and protocol tailored to you not only best meets your wishes and needs, but also allows me to ensure maximum reproducibility of the desired result for you. This of course requires an appropriate treatment technique and the highest product quality.

The treatment costs are based on the location and number of regions to be treated. If a dose increase is necessary or advisable after a treatment, I generally do not charge you. The same applies to minor corrections, e.g. in the unlikely event of an asymmetry. However, as the final result can only be assessed once the full effect has set in (after about two weeks), a correction is only advisable after this period.

In some practices, you are treated on a “price per amount” or “price per syringe” basis. Often the Botox is already drawn up in a defined amount in syringes and stored in the freezer to be thawed shortly before the treatment (“frozen Botox”). At first glance, it seems fair and transparent to pay for a defined dose, as an experienced treating doctor can certainly estimate for a given region how much dose, and thus how many units of Botox, will be needed for the planned treatment or for a result. However, if the effect is insufficient and a follow-up treatment is necessary, you usually have to pay again, because you paid for a certain dose and not for the best possible result. In your own interest, it is therefore better to ask and decide for yourself which treatment concept convinces you personally.

Frequently asked questions about Botox treatment

Which areas are most frequently treated with Botox?

Every person has individual facial expressions of varying intensity. Depending on predisposition, lifestyle and skin condition, these sooner or later lead to corresponding changes in the face, which in turn give rise to different wishes and needs for change. For one person, even superficial laughter lines (crow's feet) may be bothersome; for another, they are attractive.

The vertical frown lines between the eyebrows (glabella) or horizontal forehead lines, on the other hand, are usually perceived as bothersome, as they suggest, for example, a grumpy facial expression to others. In general, the forehead, the glabella region and the outer eye area are among the most frequently requested regions for correction with Botox. In addition, the outer eyebrows or the corners of the mouth can be specifically lifted, vertical lines around the mouth (lip lines) reduced, or vertical bands on the front of the neck (platysma bands) weakened. The face has over 20 paired facial muscles whose activity is largely in balance. Depending on predisposition and individual facial expressions, certain muscles can gain the upper hand. The associated persistent overactivity, facial patterns practiced over decades, but also environmental influences, chronic nicotine consumption, normal aging processes and other factors ultimately lead to structural damage in the skin and thus to characteristic and usually unflattering changes such as wrinkles. Treatment with botulinum toxin can therefore not only reduce wrinkles but also even out unfavorable muscular imbalances and asymmetries, which can have a very positive effect on facial expression.

How long does the effect of Botox last?

As a rule, an effect of 4–6 months can be expected. The first effects appear, varying from person to person, after just 3–6 days; the full effect sets in after about 14 days, which is why a treatment result can only be meaningfully assessed after this time. Strong muscular or facial activity at the start of treatment, high metabolic activity or smoking shorten the duration of the effect. In very rare cases, the formation of antibodies can also lead to a loss of effect. A definitive statement about the success of the treatment can therefore never be made. In addition, you should always critically ask which product your treating doctor works with and whether your doctor prepares the botulinum toxin freshly for you or uses Botox that has been preserved or deep-frozen for days or even weeks. There are sometimes very large differences in quality, purity and duration of effect! This is consequently also reflected in the treatment costs. The decision is yours.

How does Botox treatment work?

After the consultation, a precise analysis of your face and information about how it works and possible risks, Botox treatment can take place immediately if you wish, or you can make a new appointment.
After the injection solution for the planned treatment has been prepared and the region to be treated briefly pre-cleansed, the current findings and your personal facial expressions are documented photographically. The planned injection points are then marked on your skin. I will again ask you to activate your facial expressions to the maximum before the treatment is carried out by injecting the substance through an extremely thin, low-pain cannula. You may then cool the treated region for a few minutes in a comfortable sitting position. After the treatment, depending on individual tendency, minor swelling and possibly redness may persist, which resolve within a very short time.

What are the risks of Botox treatment?

In experienced hands, the risks of Botox treatment are very manageable and largely predictable. However, it is important that you follow a few small rules which, in addition to the treatment itself, can also determine success or the risk of complications (see below). As with any injection treatment, temporary swelling, redness and, rarely, small bruises can occur at the injection site, for example, but these quickly disappear and can be covered with make-up. A temporary feeling of tightness, itching or brief sensitivity to touch are also possible. In rare cases, visible asymmetries or undesirable muscular impairments occur. These include, for example, excessive lowering of the eyebrows when treating low-lying horizontal forehead lines or a temporary weakness of the eyelid lifter when treating eyebrows that sit too low (chemical brow lift). Subtle asymmetries are always potentially possible due to differing muscle activity, but can usually be evened out or corrected without any problems. During pregnancy and breastfeeding, treatment with botulinum toxin or hyaluronic acid is not possible due to incomplete data.

What should I bear in mind before and after Botox treatment?

Before a planned treatment, I ask you to cleanse your face of cosmetic products if possible. Of course, I also provide suitable products in the practice. If you are approaching Botox treatment for the first time, treatment at the end of the week or at the weekend may make sense, so that you can go straight home in the unlikely event of a small bruise. The same applies to other injection treatments such as hyaluronic acid. If you have already had good experiences and know how your skin reacts, there is nothing to prevent immediate treatment. Here I follow your wishes.

After the treatment, I recommend cooling the treated area for a while. I also recommend not undertaking any major physical exertion for 24 hours (e.g. heavy lifting, jogging, fitness training, sauna, thermal bath, etc.), as on the one hand this can provoke swelling of the treated region and, in the worst case, the freshly injected Botox can shift (diffuse) into neighboring regions where no effect is desired. Keep your head as upright as possible for the first few days, avoid bending over and avoid massaging the freshly treated region. Special exercises such as consciously activating the treated region are not necessary. For 12 hours, you should not treat or cover the treated region with cosmetic products, as a certain risk of contamination and inflammation is conceivable.

Are there any special features of treatment with botulinum toxin?

Unlike hyaluronic acid, treatment with botulinum toxin may only be carried out by a doctor. Accordingly, it is my duty to inform you not only about how it works and alternative treatment methods but also about possible risks and complications. Treatment is therefore not possible without your written consent. As a specialist in plastic and aesthetic surgery, I am thoroughly familiar with the anatomical structures of the face and their function, which is why I can only recommend that you have the treatment carried out by a medical colleague with many years of experience.
